Center ISD Announces Flu Shots Offered to Students on October 24

Update:

October 25, 2018 - Due to the overwhelming response to the district wide flu shot clinic on October 24, Aurora Concepts will be opening up 2 more afternoons to give student flu shots at school.

Tuesday, October 30: Center Middle School
Wednesday, October 31: Center Intermediate School

For students at other campuses who did not receive their flu-shot at the previous clinic and still wish to receive one, let the school nurse for the appropriate campus know ASAP. These will be given following the clinics at the Middle School and Intermediate School.

The deadline for registration and turning in completed consent forms is Monday, October 29. In order to complete insurance verification and to ensure that enough supplies are available the day of the clinic, it is important that this deadline be met.

October 17, 2018 - Aurora Concepts will be holding their annual Flu Shot Clinic for the entire District on Wednesday, October 24, 2018. They will be starting at F.L. Moffett at 8:15 a.m., and then proceeding to the other campuses in the District.

You do not have to be present when your child receives his/her immunization, but students WILL NOT be able to receive any immunizations without prior written consent from his/her parent/guardian. The consent form is attached.

If you are unsure if your child returned his/her consent form please contact the school nurse or print and fill out the attached document.  It is important that the insurance section of the consent form be filled out in its entirety.

If applicable, and you wish for your child to participate in the school immunization clinic, you can either file on insurance OR, if your child does not have insurance, pay $5.00 per immunization due at the time of service. If you are underinsured (you have insurance, but insurance does not cover immunizations) you must still complete the insurance section.

If there are any additional immunizations that your child needs, please make arrangements with the school nurse on your child’s campus ahead of time in order to assure that there are enough vaccines and supplies available to accommodate your child the day of the clinic.
