January 5, 2015 - The San Augustine County Veterans Transportation Assistance Program provides transportation reimbursements for San Augustine, Shelby and Sabine County Veterans, their surviving spouse or volunteers providing transportation to any Veteran requesting travel assistance to medical appointments.
(Please note: Veterans who are currently receiving travel assistance are not eligible for this program.)
This service will provide mileage reimbursements to and from the SA County Courthouse:
• Charles Wilson VA Clinic in Lufkin, TX (95 miles round trip - $54)
• Overton Brooks VA Medical Center in Shreveport, LA (188 miles round trip - $106)
• The Michael E. Debakey VA Medical Center in Houston, TX (335 miles round trip - $189)
Important Program Guidelines:
1) Veterans may use this service two times per week.
2) There are NO limits on the number of weeks service can be used.
Reimbursement logs are provided – and MUST include the following:
• The service number associated with the veteran being transported
• Starting and ending odometer readings
• Date of travel
• Medical appointment location – please see above
Please provide a copy of Serviceman’s DD-214, discharge certificate or VA eligibility card and a copy of the appointment letter to the Veterans Service Office.
Reimbursement logs are to be submitted to Sherry Riley at San Augustine Courthouse or Shelby County Historic Courthouse.
Reimbursement logs will be submitted to the County Treasurer for approval and issuance of payment.
Note: Reimbursement calculations based on the current Texas Comptroller rate for automobile mileage.
