April3, 2025 - We are excited to partner with Good Side Health for our UIL Sports/Extra Curricular Activities Physicals this year. A team of medical professionals will be at CHS on MONDAY, APRIL 28.
We need pre-registration completed by parents by MONDAY, APRIL 21. The company will staff based on pre-registration and are not able to take walk-up registrations.
There are two options to select from. You can choose the "Sports Physical Option" for $25. Or, if you so chose, you can select the "Well Child Check" option and fill out your insurance information to have your insurance billed.
This is for all Center ISD students grades 7-12 planning to participate in Athletics or Band for the 25-26 school year.
