May 6, 2021 - Next Monday morning, May 10th, Joaquin ISD will be providing athletic physicals to all incoming 7th-12th grade athletes. All athletes were sent home with a physical form. The physical will cost $15. There is a link below where you can print out another copy of the form if needed.
Please send your child along with their completed/signed physical form and $15 Monday morning. Athletes are required to have this form on file to be able to participate in sports for the upcoming school year. Click here for the form
